Snoring has long been a prevalent condition upsetting many people’s lives. Nearly 50% of middle-aged adults have some episode of snoring in their lifetime. Let’s see, that is about 75 million Americans. Talk about making a lot of noise!

Some people are regular snorers; others not so much. The degree of loudness also varies with each individual. The odd thing about it is that snoring is accepted as normal. People don’t often think about it as anything more than an inconvenience maybe because it’s difficult to find ways to stop snoring. We tend to think it is an accepted part of growing older.

Well how many of us have slept with someone that keeps them up all night with their snoring? It can be a nightmare. And, no one talks about the consequences it has on a person’s health. There is more evidence that snoring is not just a nuisance but is connected to increased morbidity rates including cardiovascular disease, impaired cognitive function, sleep apnea, diabetes and obesity. Not to mention not being able to stay awake at your day job and the accidents that can happen as a result!

Several factors can influence the snoring as well as its intensity such as:

- Age

- Alcohol

- Prolonged inactivity

- Tobacco smoking

- Sedative drugs

- Narcotics

- Sleep position, and

- Stages of sleep

Sleep position, sleep architecture, and breathing routes provide some clues as to what structures are restricting airflow which in turn leads to increased respiratory resistance, or snoring. For instance, if you are sleeping on your back with your mouth closed, the most likely airway obstruction is the result of your tongue falling into the back of your throat caused by gravity.

If you are sleeping on your back with your mouth open, the lower jaw and tongue as also subjected to gravity and this can cause a subsequent collapse of the airway which magnifies the obstruction. Sleeping on your side lessens the influence of gravity but does not necessarily stop snoring.

It’s natural for people to change sleep position during the night. In response, the snoring mechanism also changes. Multiple parts of the upper airway can obstruct the airflow differently. This is one reason doctors find it difficult to recommend surgery because it’s hard to identify the exact offending structure. In part, this could be why surgery success rates for snoring are so low.

We tend to laugh at snorers and make jokes about them. But in reality, snoring is not a laughing matter. It can be a serious and even life-threatening situation.

What is Sleep Apnea?

Sleep apnea is considered a sleep disorder characterized by pauses in breathing during sleep. Each episode lasts long enough so that one or more breaths are missed. This can happen repeatedly throughout the night and the interval between breaths is typically at least 10 seconds. Clinically significant levels of sleep apnea are defined as five or more episodes per hour.

Obstructive sleep apnea (OSA) is the most common category of sleep-disordered breathing, and it requires treatment to prevent sleep deprivation and other complications including congestive heart failure. Combine OSA with being overweight, and you have an even bigger problem.

Breathing can continue to become shallower until it actually stops for a period of time, at first short periods of time that can become longer, with the cycle repeating all night long. This can lead to more serious health problems.

How is Snoring and Sleep Apnea Treated?

Some treatments involve lifestyle changes such as avoiding alcohol or muscle relaxants. You will probably be told to lose weight and quit smoking. Many people benefit from sleeping at a 30 degree angle or higher as if in a recliner because gravity has less of a chance to get at your breathing patterns and collapse your airway.

Sleeping on your side as opposed to your back is recommended but rarely solves the whole problem. Some people find benefit from various kinds of oral appliances that are used in the mouth during sleep and help keep the airway open.

Equipment called “breathing machines” like the continuous positive airway pressure (CPAP) may help also. And, there are surgical procedures that can be used to remove and tighten tissue and/or widen your airway. Of course, surgery should be a last resort.

Dr. Walter Fong in Southern California believes there currently are no products that can cure snoring. He has developed a method called the Silent Night Treatment which involves retraining your muscles that he claims has had a significant impact on snoring for many of his patients.

BIPAP: When You Can’t Wear CPAP

CPAP is an acronym that stands for continuous positive airway pressure. It is a form of therapy used for people who have obstructive sleep apnea. Sometimes the continuous pressure of CPAP is uncomfortable for the person using it. If that happens, BIPAP may be an alternative.

Though BIPAP therapy is similar to CPAP and will get the same results for the sufferer using it, it does have some differences. CPAP therapy gives one constant pressure for both inhalation and exhalation when breathing. BIPAP has two settings, instead of a constant one. When a person breathes in the pressure is higher and when they exhale, it is lower.

Some patients have difficulty with the pressure of CPAP, especially when exhaling, so BIPAP can be a solution for that.

Another difference between BIPAP and CPAP is that BIPAP is used to treat more than just obstructive sleep apnea. Some BIPAP machines can be set to give a person a breath. This would be used with central sleep apnea or complex sleep apnea, when the patient is having apneas because the brain signal does not make it to the chest muscles properly.

It is also a good device for a person with chronic obstructive pulmonary disease (COPD). People with COPD have low oxygen levels during the night when they sleep. Their lungs are stressed. A BIPAP unit can help them to breathe more comfortably, while also sustaining their oxygen levels at a normal percentage.

Some of the new BIPAP machines have a feature called Biflex. This is a comfort setting which provides a smoother transition from the end of inspiration to the beginning of expiration.

Complications from BIPAP are similar to those of CPAP. There may be mask discomfort. It is crucial to find the best fitting mask or sores may develop at certain pressure points on the mask.

Bloating from swallowing air can be another complication, but does not happen very often.

Your doctor can help decide which form of PAP therapy will be best for you by taking your history and the results of your tests into consideration.

There is a substantial difference in snoring and sleep apnea but, ironically, the treatments are much the same. It is first important to differentiate the two terms.
Snoring is nothing more than a vibration caused from fatty tissue rubbing against the back of the throat during sleep. It is often the cause of sleep loss, aggravation and inconvenience. A Fatal Sleep Disorder

Sleep Apnea is a serious sleep disorder that can be life threatening and at times fatal. People with sleep apnea often fall asleep normally; however, once asleep their ability to breathe is blocked. Usually this inability to breathe is caused by the muscles in the throat relaxing too much and collapsing into the airway. The body then sends a signal to the brain that breathing has temporarily been blocked. This causes the person to wake up and start breathing again.  The cycle of interrupted breathing can occur many times throughout the night. These episodes can occur up to 50 times an hour and last for ten seconds or longer. Often the person that suffers from this sleep disorder is unaware that anything is happening to them. They can not understand why they always feel tired during the daytime. The most common form of this sleep disorder is obstructive sleep apnea, known as OSA. Some sufferers of obstructive sleep apnea also suffer from cental sleep apnea. This sleep disorder is then called mixed sleep apnea. It is believed that obstructive sleep apnea affects between 18 – 20% of the adults in the United States. If this sleep disorder is left untreated in can become life threatening and in rare cases fatal. It is the underlaying cause of illnesses such as heart disease, stroke, pulmonary hypertension and systemic hypertension. There are several method of treatments for sleep apnea, both surgical and noninvasive. The first line of therapy for someone suffering from moderate to severe sleep apnea is called positive airway pressure. Known as PAP, positive airway pressure is a noninvasive form of treatment. A machine delivers a constant flow of air through a mask that is worn while sleeping. The force of the air flow must be determined by a sleep technician during an overnight sleep study. There are three types of positive air pressure therapy, CPAP, BiPAP and AutoPAP. Dental devices are also used to treat this sleep disorder in mild to moderate cases. Dental devices fall into two general categories: mandibular, or lower jaw, advancing devices or tongue retaining devices. Mandibular devices are used most often. They attach to the upper jaw and pull the lower jaw and base of the tongue forward. This shift in position keeps the airway open. Medications are generally not a successful form of treatment for most people with sleep apnea. However, many of them do take antidepressants and mondafinal. Supplemental oxygen is often used in conjunction with a PAP machine. Oxygen alone can not prevent the collapse of the airway or sleep fragmentation. However, oxygen can prevent the drop in the level of blood oxygen that occurs when the airway collapses.  There are also surgical treatments for obstructive sleep apnea that may be an option for some sufferers of this sleep disorder. These include uvulopalatophrayngoplasty, somnoplasty, corrective jaw surgery, palatal implants and tracheostomy. People with this sleep disorder generally find that their quality of life can improve with the proper treatment.

CPAP Machine – Care and Maintenance

If you have sleep apnea, you know that your CPAP machine is the only tool that will determine if you will spend your day cranky and sleepy or focused and cheerful. This is why you need to wear it every night. This is also a good reason why you should take care of your CPAP machine.
These machines last, on average, 12 to 14 months. But if properly taken care of, the machine can last several years. And since they’re not exactly the cheapest equipment there is, you might want to make sure that they serve you for a good number of years. The following are great tips that will help you care for and maintain the optimal performance of your machine:Proper Cleaning Schedule
It is important to wipe the whole mask and the external part of the machine every day. Other parts of the machine, however, need not be cleaned daily. The tubing, cushion, mask frame, and other parts will have to be cleaned once a week. The same is true for the filters of the machine. If the filter is the non-disposable type, you will have to replace it every six months. The disposable filters must be replaced when they have turned dark. Moreover, if you’re using a rental machine, you might have installed bacterial filters, too. These filters must be replaced every 10 to 14 days. Proper Cleaning Process
The mask, its frame, and the cushion can be handwashed with warm soapy water. Use a detergent that is mild. You should also avoid using very strong cleaning agents, such as bleach, alcohol, solutions with chlorine, and other household cleaners. After washing these parts, make sure the you let them air dry. They should also never be dried under direct sunlight. Before putting them back together, make sure that they are not damaged.
It is also possible to use the dishwasher when washing these parts of the CPAP machine. Again, it it best to use a mild detergent and to air dry the parts away from direct sunlight. When you assemble the parts that you have cleaned, you must be careful so as not to damage them.
You can also wash the tubing in warm soapy water. Before putting it back on again, you must make sure that it has no holes or splits. Any damaged part that you see must be replaced right away.
As for the filters of the CPAP machine, cleaning them once a week is also important. These foam filters make sure that you do not inhale dust and pollens in the air. You can wash the foam with mild detergent and squeeze it under running water until it becomes light-colored again. If your machine does not have disposable filters, you would still need to get rid of them after half a year.
There are machines with disposable filters and these have to be replaced after 3 months. If you see them start to get dark in color, you’ll have to replace them right away. This ensures that you’re breathing only clean air while you are asleep.

It may well require a multi-prong approach. However making sensible decisions regarding your lifestyle and diet can be a helpful first step to take.
